RN Home Health Case Manager
Start Date:

Pay Rate: $1800.00 - $2700.00
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in a home health/hospice setting
MINIMUM REQUIRED QUALIFICATIONSVT/compact RN license; BLS: 1+ year recent home health and case management experience;
Oasis experience;
No requested time off during assignment
20 weeks
SHIFT / HOURS PER WEEKTuesday - Saturday (M-F first week to facilitate orientation)
