** We are seeking an enthusiastic and skilled Home Health RN to join our healthcare team in Central Vermont! ** - Provide exceptional nursing care in home health settings - Conduct assessments and develop individualized care plans - Manage a caseload of patients with diverse needs - Complete OASIS assessments with precision and detail - Collaborate with interdisciplinary teams to ensure optimal patient outcomes - Educate patients and families on health management and safety

* Qualifications:

** - Minimum of 1 year of home health and case management experience - OASIS experience is required - Strong preference for experienced travelers - Must have NO requested time off for this contract - Current BLS/BCLS certification
