An exciting opportunity to join our integrated heart failure (HF) specialist nurse team based in Barnstaple North Devon.

Non medical prescribing is a requirement of the role and this post would suit either a band 7 competent & experienced heart failure nurse or a suitably experienced clinical pharmacist. However, for the right candidate, we can offer a development band 6 post with progression to band 7 once training requirements are met.

More detail about the role

The post holder will be predominantly based within the acute heart failure team at North Devon District Hospital, providing specialist HF care for inpatients but there is opportunity for outpatient, telephone and community follow up too. As the service is an integrated heart failure service, the post holder will be asked to work flexibly across sites and different elements of service provision.

The role encompasses diagnosis, symptom management, optimising treatment, supporting patient self-monitoring, education, and delivery of effective palliative care. There is a huge emphasis on multi-disciplinary working and the post holder will benefit from taking part in cross site clinical multi-disciplinary meetings.

This is a hugely rewarding job in a welcoming forward-thinking team. We are looking for someone who is a strong team player but who is also able to work autonomously.

Working Pattern: 37.5 hours per week. Monday to Friday 9 am to 5 pm
