33 hours to be worked flexibly on a rota basis Monday to Sunday 8am – 8pm

Our Community Service includes Clinical Nurse Specialists, Registered Nurses and Multi Skilled Healthcare Assistants working together to deliver expert end of life and specialist palliative care to patients and families in their own home.

We are looking for a dynamic, highly motivated, forward thinking registered nurse to join our community service as a Community Palliative Care Staff Nurse. Working alongside the Clinical Nurse Specialists and Multi Skilled Healthcare Assistants (MSHCA), you will take responsibility for a caseload of patients who require follow-up care and rapid interventions  will also have oversight of planned packages of care delivered by our team of MSHCA, supporting them to safely deliver personal care at home throughout our 7 day service.

Your clinical knowledge and assessment skills will be used to support patients who call for urgent assistance. You will work across the full week, and weekends be responsible for supporting the MSHCAs.

Experience of working in the community or demonstrable understanding of the coordination and integration of community services. You will need to be able to work within a busy and close-knit team whilst also demonstrating a degree of autonomy in your practice. You will be committed to person centred care and evidence-based practice.

We support continued professional development within the team and you will be encouraged to seek out opportunities to learn and develop. We will provide a tailored induction and training programme to support your needs and integration into our service.

You must be able to travel across our geography and be able to deliver bedside care.
