We are looking for an enthusiastic and committed Assistant Social Care Practitioner to join our Adult Social Care team. The role is full‑time, permanent and peripatetic across local authority services. You will work as part of a skilled team and will be supported to manage an individual caseload autonomously.
Key responsibilities include completing service user and carer assessments, undertaking reviews, coordinating care planning, commissioning and provisioning services, and ensuring early intervention and strengths‑based outcomes.

- Coordinate care planning, commissioning, and service provision to meet desired outcomes.
- Manage priority tasks, identify risks, and organise workload in line with statutory frameworks.
- Work independently and as part of the team to meet service demands.
- Adhere to council policies, procedures and relevant social care legislation.
- Experience working in health and social care settings.
- Strong organisational and prioritisation skills.
- Ability to identify risks and adapt to statutory requirements.
- Good communication skills and an appropriate standard of spoken English.
- Enhanced DBS Check.
