Overview

Are you a registered MH nurse or about to qualify as a RM? The Enhanced MH Team West in Barnsley invites you to join a multidisciplinary team delivering high quality face to face care using the Flexible Assertive Community Treatment (FACT) model.
The successful candidate will be central to the service user’s recovery journey, working with other agencies and alongside family & carers to keep the person at the centre of all we do. We offer flexible working hours between 08:30–19:00 (Mon–Fri) and 9:00–17:00 on weekends & Bank Holidays. We are family friendly and provide agile working equipment. The service is open 365 days a year and prides itself on excellent teamwork.

Main duties of the jobThe successful candidate will:
- Manage a caseload of around 25 people, arrange care, liaise with other professionals and agencies, and review care plans and risk assessments in a recovery‑focused, collaborative manner with service users and their carers at the centre of all activity.
- Work within the Flexible Assertive Community Treatment (FACT) model and participate in a daily shift rotation which requires delegation, prioritisation, overseeing the daily morning meetings, managing the team diary and carrying out urgent visits.
- Be an efficient organiser, manage their own workload, and be flexible to meet service needs; arrange supervision in line with professional registration.
The team uses the FACT model with a daily shift coordinator on a rota. The post holder will delegate and prioritise work, oversee daily operations, and may be required to undertake urgent visits. Supervision and professional development are part of the role.
Responsibilities and duties (Key Result Areas)- Work as a member of an integrated multidisciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating interventions to address identified needs.
- Maintain knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered (e.g., CPA, MHA).
- Contribute to service development within the clinical governance structure and Trust policies, adhering to NHS guidance and statutory registrations.
- Assess needs, identify problems, devise a plan of care in partnership with the person and carers, implement the plan to a high standard, review effectiveness, and take action where needed.
- Competent in care and risk management as defined by the Care Programme Approach; collaborate with clients, carers and multi-agency partners to maintain mental health and functioning.
- Manage enquiries and direct referrals per Enhanced CMHT Operational Policy and participate in the team duty rota; maintain professional registration and CPD commitments; safe custody, transport and administration of medicines where appropriate.
- Provide peer supervision and support; supervise junior staff in maintaining client and Trust records; participate in clinical audits and CPD; engage with evidence‑based practice and values‑based appraisal; participate in personal development opportunities.
- Participate in safeguarding and related training; ensure compliance with Trust policies.
