Community Mental Health Nurse

If this is you, then the Enhanced MH Team West in Barnsley wants you to come & join us.

We are a multidisciplinary team that provides high quality face-to-face care utilising the FACT model (Flexible Assertive Community Treatment) which is central to keeping people well and out of hospital.

The successful candidate will be the key person in the service users' recovery journey, working with other agencies and alongside family & carers, keeping the person at the centre of all we do.

We offer flexible working hours between 08:30 – 19:00 Monday to Friday and 09:00 – 17:00 on weekends & BH. We are family friendly & all staff have agile working equipment. We are open 365 days a year & pride ourselves on excellent team working.

Main duties of the job

The successful candidate will manage a caseload of around 25 people, and will be responsible for arranging care, liaising with other professionals and agencies, managing and reviewing care plans and risk assessments ensuring these are done collaboratively in a recovery focused manner, keeping the person and their carers at the centre of all we do.

The team uses the Flexible Assertive Community Treatment Model, and we have a daily shift coordinator on a rota system which requires the successful candidate to delegate and prioritise work, oversee and facilitate the daily morning meetings, manage the team diary and carry out any urgent visits.

The successful candidate will need to be an efficient organiser, managing their workload and be flexible to meet the needs of service. They will need to manage their own requirements for supervision in line with their professional registration.
