- Lead and support the hospital social work team, ensuring excellent service delivery.
- Attend daily meetings with health partners to discuss and plan patient discharge processes.
- Balance individual needs with healthcare pressures in line with integrated care board partnerships.
- Carry a small caseload alongside your line management responsibilities, maintaining a hands‑on approach.
- Collaborate efficiently with health partners to ensure safe and timely patient discharges.
- Undertake Care Act, Capacity, and Best Interest Assessments as required.
- Significant hospital social work experience with adults and comprehensive knowledge of discharge processes.
- Proven ability to manage and supervise social work staff, fostering a cooperative team climate.
- Expertise in collaborative working to ensure safe, timely discharges in partnership with health systems.
- Strong proficiency in IT systems including Liquid Logic and Excel.
- Thorough understanding of relevant legislation such as the Mental Capacity Act 2005, Human Rights Act 1998, and Care Act 2014.
- Competence in working with complex needs including substance misuse, homelessness, and mental capacity issues.
- Confidence in decision‑making, staff supervision, and caseload management.
