Senior Community Memory Nurse

The role is to become a member of the multi‑disciplinary team working together to assess, review and offer pharmacological and non‑pharmacological treatments for people with Dementia, with a view to obtaining a diagnosis if appropriate.

The client group will be people referred to the service due to cognitive decline.

The purpose of the role is to complete initial assessments for this client group to enable a diagnosis to be made, to offer post‑diagnostic information and support to the person with dementia and their carers.

Also, part of the role is working with people whose presentation may have changed due to a variety of reasons including Behavioural and Psychological Symptoms of Dementia.

The candidate should have a good understanding of the symptoms associated with dementia and how this may affect people and their day‑to‑day lives and the additional support that they and their carers will require.

The candidate may be required to work with people in their own homes and/or people in 24 hour care settings.

Main duties of the job

The successful candidate will work within the multi‑disciplinary team to assess people experiencing dementia in both the community and care home settings.

Once a dementia diagnosis has been formulated within the MDT, the candidate may be required to deliver that diagnosis to the client: offering post‑diagnostic information relevant to that individual and supporting the person with the diagnosis and their families/carers.

Providing ongoing care coordination for individuals on your caseload, including people subject to the Care Programme Approach or equivalent clinical guidance under your care.

Offer pharmacological and non‑pharmacological treatments, review these and offer advice and support as necessary. Providing ongoing review of individuals prescribed anti‑psychotic treatments in line with a local pathway.

To supervise junior members of staff within their role.

Joint working/liaising with statutory health and third sector services who may be involved in the client care.

Work with the whole team to find new ways of working and innovative practice to meet the needs of the client and their carers.
