An exciting opportunity is available for an Advanced Social Work Practitioner to join the Leaving Care Team in Barnet, London. This ongoing role offers a competitive hourly rate of £37.54 and the flexibility to choose between full-time and part-time hours. Core working hours are Monday to Friday, 9am–5pm, with alternative options such as 8am–6pm across four days, 8am–4pm, or 9am–6pm, allowing you to fit your work schedule around your lifestyle.

What You Will Do

Provide tailored support to care leavers aged 16 to 25, adapting your approach based on their education, employment, and personal circumstances.

Assist young people in obtaining suitable accommodation and guide them through benefit claim processes.

Develop and review pathway plans focusing on education, employment, life skills, and long-term independence.

Present cases to the Commissioning Panel for accommodation funding where required.

Manage weekly living allowance arrangements and provide financial guidance to care leavers.

Undertake assessments to determine individual support needs and advocate effectively on their behalf.

Ensure all statutory responsibilities for care leavers are met and that practice complies with relevant legislation.

Maintain consistent contact with young people to build stable, trusting relationships.
