Community Sister​/District Nurse

Job Summary

We are seeking a Community Sister to join our Teesdale Community Nursing Team. The role covers the Teesdale Locality and surrounding areas, operating from an office base. The team works 8 am – 9 pm, 7 days a week on a duty roster, providing high‑level care and assessment to unplanned patients requiring urgent community resus (UCR) and holistic nursing care to adults in their own homes or residential care settings.

• Provide nursing care, including wound care, medication administration, palliative care, bowel and catheter management.
• Assess patient care needs, develop and review care plans, and deliver care.
• Work independently within the wider nursing team and report to the Team Lead.
• Support patient safety and clinical effectiveness by providing expert evidence‑based advice.
• Supervise, line‑manage and train staff, nursing associates and HCAs.
• Respond to urgent care demands and support planned care when required.

• 1st level registration with the NMC (Adult).
• Professional knowledge of community nursing (degree or equivalent).
• Ability to work flexible hours and travel independently.
• Exceptional communication skills, able to convey complex and sensitive information with empathy.
• Experience supervising staff and providing clinical supervision.
• Works within professional codes of practice and NHS policies.

• Community specific skill set.
• Experience in HR management, audit/research or clinical supervision.
• Physical skills course or equivalent, non‑medical prescribing, management and leadership course.

• Essential – Experience working as a nurse in a community setting/team.
• Desirable – Experience managing a team, audit/research, clinical supervision, evidence of ongoing nursing development.

• Essential – 1st level registration with the NMC (Adult); professional knowledge of community nursing and supplementary diploma‑level training लड़की? or courses.
• Desirable – Physical skills course, non‑medical prescribing, management and leadership course.

• Band: 6
• Salary: £38,682 to £46,580 a year pro‑rata
