We’re seeking a highly motivated and experienced home visiting Physicians & Nurse Practitioners (NP) to join our team as a full-time or part-time contractor. In this role, you will be a vital part of our care model, providing a comprehensive approach to the management of chronic conditions for our home-bound patients. You will have the unique opportunity to deliver a high level of care in the comfort of our patients' homes, building strong, meaningful relationships with them and their families.

Key Responsibilities- Provide comprehensive medical care to a panel of home-bound patients.
- Perform a wide range of services typically offered in a physician's office, including physical exams, diagnostic testing, and health screenings.
- Develop and implement individualized care plans to manage chronic conditions such as diabetes, hypertension, COPD, and heart failure.
- Collaborate with the care team to ensure seamless transitions for patients from the hospital to the home setting, with the goal of reducing hospital readmissions.
- Educate patients and their families on their conditions and treatment plans to promote self-management and adherence.
- Maintain accurate and timely electronic health records (EHR) documentation.
- Serve as a compassionate and trusted point of contact for our patients, addressing their medical needs and concerns with empathy and expertise.
- Current and unrestricted Nurse Practitioner (NP) or Physician license in the state of Illinois.
- Experience working with a geriatric patient population is highly preferred.
- Experience in home healthcare is highly preferred.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a patient caseload effectively.
- Experience with electronic health records (Practice Fusion preferred).
