  • Key responsibilities:

    Deliver high‑quality, patient‑centered care within the LPN scope in long‑term care settings. This includes administering medications and treatments per care plans, monitoring vital signs and response to therapies, assisting with activities of daily living, wound care, and infection prevention, documenting accurately, and communicating changes to the nursing team. You’ll collaborate closely with RNs, physicians, therapists, and families to update care plans, educate residents and caregivers, and participate in rounds and care coordination to ensure safety and comfort.
