Nurse Care Manager

Job Description

The Nurse Care Manager provides professional, patient-centered nursing care in an ambulatory setting, adhering to a multi-disciplinary plan of care and established policies and procedures. This role involves providing direct nursing and telephonic services to patients and families in accordance with the standards of practice for Ambulatory Care Nursing. The Nurse Care Manager collects and analyzes patient data, assists in the development and maintenance of care plans, and evaluates intervention outcomes.

Documentation and communication of interventions and outcomes are required per professional and organizational standards. Additionally, the Nurse Care Manager delegates tasks to other licensed and unlicensed care providers as appropriate and ensures compliance with regulatory body standards.

Responsibilities

* Utilize assessment skills to evaluate patient needs and formulate a plan of care as part of the multi-disciplinary care team.

* Facilitate individual or group nursing visits with patients to address chronic illnesses, providing education about disease management and treatment.

* Identify patients at risk for poor outcomes and provide additional outreach and frequent follow-up.

* Provide complex case management, including chronic disease case management, critical diagnosis care coordination, transition care management, high-risk clinical tracking, complex medication management, and system utilization.

* Conduct medication reconciliation and adherence counseling, assessing patient knowledge and barriers to adherence and self-care.

* Collaborate with the multi-disciplinary team to address both physical and psychological health aspects for comprehensive patient care.

* Coordinate with the Transition of Care Team to ensure safe and effective transitions of care.

* Assist patients in developing self-management goals and coordinate follow-up as needed.

* Teach patients self-care skills, including medication administration and preventative health screening.

* Delegate clerical and non-nursing clinical tasks to assistive personnel as appropriate.

* Document both incoming and outgoing calls and communicate effectively with other team members to ensure continuity of care.

Essential Skills

* RN certification

* Experience in case management and managed care

* Proficiency in patient education and referrals

* Knowledge of electronic health records

* Ability to work with computers in word processing and database applications

* CPR certification

Additional

Skills & Qualifications

* Graduate from an Accredited School of Nursing

* Current Maryland Nursing License

* Preferred:
One year of nursing experience in an ambulatory or acute care setting

* BSN preferred

* Significant related experience with populations served may substitute for the above requirement

Work Environment

This position is based at the Mt. Vernon Center, located in the heart of Mt. Vernon in the historic Monumental Life Building. The facility houses PEDs, Adult Primary Care, OB/GYN, and more at their downtown site. The position offers a standard Monday to Friday schedule with no weekend shifts. The work environment is dynamic, requiring flexibility and adaptability to a changing environment.

Job Type & Location

This is a Contract position based out of Baltimore, MD.

Pay and Benefits

The pay range for this position is $40.00 - $45.00/hr.
