Summary

Licensed Practical Nurse (LPN) – Hall 2 (Long-Term Care / Memory Care)

Location: Levindale Hebrew Geriatric Center & Hospital
Department: Hall 2 (Long-Term Care / Memory Care)

Unit Summary

Hall 2 is a 50-bed long-term care and memory care unit that provides residents with daily activities and a comprehensive range of services and support. This unit serves residents with varying levels of acuity, from high-functioning individuals to those who are bedbound. Common diagnoses include diabetes, cardiac conditions, dementia, and behavioral health needs. Staff members on Hall 2 are expected to deliver patient-centered, compassionate care while supporting both medical and cognitive needs.

Summary

Under the supervision of the Registered Nurse and/or Director of Nursing, the Licensed Practical Nurse (LPN) utilizes a general understanding of nursing principles and basic physical assessment skills to develop and implement individualized nursing care plans. The LPN supports daily unit operations on a per-shift basis and may assume leadership or charge responsibilities as assigned to ensure safe, effective, and compassionate resident care.

Duties & Responsibilities
  • Provide direct nursing care to long-term care and memory care residents in accordance with established care plans

  • Perform basic physical assessments and monitor resident status, reporting changes in condition

  • Administer medications and treatments as prescribed and within scope of practice

  • Support residents with chronic conditions and cognitive impairments, including dementia and behavioral needs

  • Accurately document care and observations in the electronic medical record

  • Collaborate with interdisciplinary team members to promote resident well-being and quality of life

  • Support daily unit operations and workflow

  • Serve in a leadership or charge role as assigned

  • Adhere to safety standards, infection control practices, and regulatory requirements

Minimum Qualifications
  • Graduate of an accredited school of nursing (diploma program)

  • Current Licensed Practical Nurse (LPN) license in the State of Maryland

  • AHA BLS Certification required

  • Less than one (1) year of nursing experience required

  • Formal working knowledge equivalent to an Associate’s degree (two years of college)

  • Knowledge of nursing practices appropriate for long-term care and memory care populations

Required Skills & Competencies
  • Basic computer proficiency

  • Experience or familiarity with:

    • Synquest

    • IV Pumps

    • Philips Monitoring System

    • Accu-Chek

    • EKG Machine

    • Specialty Bed Operation

  • Ability to provide compassionate care to residents with cognitive and behavioral health needs

  • Strong communication, organization, and teamwork skills
