MUST:

• Licensed as a Registered Nurse in the state of Maryland
• 4+years of experience as a Registered Nurse
• Knowledge of Patient Centered Nursing Care
• Strong clinical skills and knowledge
• Administering injections, handling wound care and triaging patients
• Performing various clinical duties
• Creating treatment plans
• Providing patient education
• Bachelor's Degree in Nursing
• CPR certified

DUTIES:

• Provide patient nursing care in the ambulatory setting
• Provide direct nursing to patients
• Perform triage and provide support by phone
• Collect and analyzes patient data
• Document phone calls
• Create and develop care plans for patients
• Document care plans and interventions
• Evaluate patient needs
• Facilitate nursing visits and provide patient education regarding available treatments for chronic diseases
• Provide outreach to patients
• Assist with transition care management
• Assess patients, assist with medication administration and assist with self-care
• Triage patients in walk in clinic
