Registered Nurse (Care Management)

Washington, DC.

Pay: $90,000 – $100,000 per year plus $2,500 signing bonus.

LMK Recruiting is partnering with a growing care management organization supporting older adults throughout Northern Virginia. We’re hiring Registered Nurses who are passionate about advocacy, coordination, and making a real impact in the community.

• Coordinate and oversee personalized care plans.
• Advocate for clients across home, medical, and care facility settings.
• Communicate with healthcare providers, families, and care teams.
• Conduct field visits (approx. 90% of the role).
• Participate in occasional after-hours or weekend on-call rotation.

• Experience in home health, care management, or senior care preferred.
• Reliable transportation and comfort driving in NOVA/DC Metro.
• Strong communication, adaptability, and accountability.
• Passion for supporting older adults.
• Bachelor’s degree + RN experience encouraged – master’s degree not required.
