- Uses advanced clinical and theoretical knowledge to identify acute and/or chronic health problems through a comprehensive, age-appropriate assessment of patients with substance use disorders.
- Provides evidence-based care and provides patient education for self-care and substance use treatment that is in support of the patients ongoing acute or chronic disease management.
- Effectively uses resources.
- Prioritizes and organizes work to meet the changing priorities of the functional unit and/or cost center.
- Provides care coordination and medical decision-making regarding patient care.
- Triages phone calls and electronic advice requests from consultants and participates in ongoing medical assessment.
- Assesses lab results and implements intervention in consultation with primary providers and medical director.
- Provides guidance and direction to other staff where applicable to promote quality, safety, and excellence in care. Analyzes findings and promotes, as appropriate, improvement in the management of patient care.
- Identifies care continuity needs of patients.
- Participates in quality improvement/patient safety projects.
- Coordinates care for a group of complex patients to achieve clinical and organizational outcomes.
- Will work with the ACC Nurse Practitioners to create coverage schedule such that there are not gaps in care on the SUDs Consult Service or in the Ambulatory Clinics.

- Advanced clinical skills in specialty area or equivalent.
- Successfully completes all training courses required for access to Epic and other clinical software systems. General computer knowledge and capability to use computer.

- Ability to incorporate into practice theoretical concepts and advanced clinical skills with patients, families, and interdisciplinary health care team members.
- Ability to analyze data to synthesize and summarize information and to make judgments regarding patient care.

We are seeking a
Substance Use Consultation Service Nurse Practitioner
who will support the Johns Hopkins Hospital (JHH) Substance Use Consolation Service.
The Substance Use Consultation Service Nurse Practitioner works collaboratively within a multidisciplinary health team to optimize team performance and patient outcomes and is responsible for providing comprehensive and age-appropriate consultative care to patients with substance use disorders (SUDs) Substance Use Consultation Service Nurse Practitioner educates patients/families and/or treats SUDs. The Substance Use Consultation Service Nurse Practitioner assists in the provision of post JHH treatment plans including the After Care Clinic (ACC) practice located in the Johns Hopkins Outpatient Center (JHOC).
The Substance Use Consultation Service Nurse Practitioner will additionally provide comprehensive and age-appropriate care to both well and/or ill patients in the ACC. The Substance Use Consultation Service Nurse Practitioner practices within the guidelines of the unit department, and institution and in accordance with the Nurse Practitioner - Physician Written Agreement required and approved by the State of Maryland.
