- Admissions/Case Manager Home Care
Home Care Maryland is seeking a dedicated and detail-oriented Admissions Nurse / Case Manager to provide exceptional in-home clinical assessments and case management for patients across Baltimore City and Baltimore County.
This role is vital in initiating skilled home care services, ensuring all patients receive comprehensive, compassionate, and high-quality care from the very beginning. As the first clinical contact for many of our patients, the Admissions Nurse will function as a key clinical liaison between patients and physicians, complete thorough OASIS assessments, and develop individualized plans of care tailored to each patient’s unique needs.
Key Responsibilities:
- Conduct timely and accurate patient admissions in accordance with regulatory standards and Home Care Maryland policies.
- Complete comprehensive OASIS documentation and initiate the plan of care based on assessment findings.
- Perform initial and ongoing assessments of patients' physical, psychosocial, and environmental needs.
- Confirm clinical assessment and care needs with the referring physician.
- Identify the need for additional skilled services and coordinate care accordingly.
- Provide clinical supervision for Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) in accordance with Maryland regulations and company policies.
- Ensure a patient- and family-centered approach to care throughout the admission and case management process.
Qualifications:
- Current Registered Nurse (RN) license in the State of Maryland.
- Current BLS (Basic Life Support) certification for healthcare providers (American Heart Association).
- 2-3 years of experience as a Registered Nurse required.
- Prior experience in home health care preferred.
- Proficient clinical assessment and documentation skills, including experience with OASIS preferred.
- Positive attitude, strong communication skills, and the ability to work independently.
