Nurse Practitioner
The Registered Nurse (RN) Medical Case Manager coordinates healthcare services for children in the Baltimore City Department of Social Services (BCDSS) foster care, ensuring timely access to medical and mental health care. This role monitors health needs, supports child welfare staff and caregivers with education and resources, and collaborates with a multidisciplinary team to establish and maintain medical homes. Strong organizational, communication, and problem-solving skills, cultural sensitivity, and compassionate care are essential.
ResponsibilitiesCase Management
- Coordinate holistic healthcare for children in the MATCH Program by partnering with the Baltimore City Department of Social Services (BCDSS) foster care program, foster parents, biological parents, managed care organizations, and healthcare providers.
- Ensure the medical and mental health care needs of children in foster care are met in a timely manner.
- Assess children at risk for potential medical and mental health complications and monitor closely.
- At registration for children sheltered with BCDSS, identify all immediate medical needs to begin health care coordination.
- Review and assess all initial medical documentation from all mandated foster care exams.
- Make recommendations in determining the appropriate continuing care team.
- Work with Program Manager to conduct weekly case transfer meetings.
- Provide recommendations, resources, and education to caregivers and child welfare workers.
- Assist foster care families with navigating the Health Choice system in establishing and maintaining a medical home for children in the BCDSS foster care system.
- Prioritize care and contacts based on urgency, including the ability to multi-task with accuracy.
- Document case management assessments, medications, treatment plans, activities, and resolutions in BCDSS and the HCAM Program database.
- Attend weekly team meetings to review case assignments and transfers.
- Participate in all meetings as required by BCDSS.
- Participate in multidisciplinary team meetings, inter-agency meetings and conferences.
- Submit timesheet timely and accurately.
- Complete all training required by Health Care Access Maryland and funders.
- Additional duties as assigned.
- May be required to work beyond normal working hours based on business needs.
- A bachelor’s degree in nursing from an accredited college/university
- Current licensure as a Registered Nurse in the State of Maryland
- Relevant clinical experience providing services to families and children, preferably in a public health/community setting
- One-year clinical nursing experience
- Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365)
- Organizational skills with the ability to meet a demanding workload
- Detail oriented to complete requirements of files and contract compliance
- Sensitivity to cultural and socioeconomic characteristics of the population served

Base pay range: $73,000.00/hr - $73,000.00/hr this range is provided by Health Care Access Maryland. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.
