Overview

Med Star Health is looking for a Case Management Assistant to join our team! We are looking for someone with strong verbal and written communication skills. Proficiency with computer and database management. Highly detail-oriented, ability to navigate through complex health systems. Proficient with Microsoft Teams. The Case Management Assistant will implement the plans and interventions defined by the Case Managers and Clinical Social Workers. Under the case management staff's direction, performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting. Assists in administering Case Management functions include organizing workflow, communications, links to community resources, transportation, and other duties to help facilitate discharges.

Provides clerical and support functions to the case management staff members with the goal of promoting expeditious patient transfers and/or discharges. Maintains a high degree of confidentiality with sensitive patient information.

Responsibilities
  • Delivers patient notification letter as needed—Notice of Observation Status, MOON, Medicare Important Message, HINN and others. Obtains the appropriate signature and uploads it into the electronic record. Escalates issues to the department manager/director as needed.
  • Assists Case Management team with post-Acute care placement (Acute Rehab, Skilled Nursing, Hospice, Dialysis, Home Health, DME, Psychiatric, Long-Term Care, and LTACH placements). Responsible for following through until discharge.
  • Obtains authorizations for post-acute care (Acute Rehab, Skilled Nursing, Hospice, Dialysis, Home Health, DME, Psychiatric, Long-Term Care, and LTACH placements) and communicates information between the hospital, payor, and post-acute facilities.
  • Coordinates and provides documentation for patient transport to and from hospital and appointments. Responsible for dedicated ambulance transportation. Arrange transportation, provide documentation, track data, and follow up to confirm successful transport.
  • Maintains multiple databases to track patterns and identify potential discharge barriers to facilitate a timely discharge. Prepares discharge packets to be sent with patients being discharged to skilled facilities. Promote customer satisfaction through effective communication and Language of Caring skills.
Qualifications
  • High School Diploma or GED.
  • 1-2 years related experience.
  • Bachelors degree preferred.
  • Medical terminology knowledge preferred.

This position has a hiring range of

USD $18.70 - USD $32.72 /Hr.
