Position: DIRECTOR - OUTPATIENT CARE MANAGEMENT

Overview

The System Director of Outpatient Care Management is a senior leader responsible for the strategic development, implementation, and oversight of care management programs across all outpatient settings within the health system. This role ensures seamless, patient-centered care coordination that improves quality outcomes, reduces unnecessary utilization, enhances patient satisfaction, and supports the organization’s population health and value-based care strategies.

Responsibilities
  • Strategic Leadership: Develop and execute a system-wide outpatient care management strategy aligned with organizational goals and value-based care initiatives. Lead standardization of outpatient care management practices, workflows, and metrics across all clinics, physician practices, and ambulatory sites. Partner with senior leadership to integrate outpatient care management into broader population health initiatives.
  • Oversight and Operations: Oversee care coordination, transitional care, chronic disease management, and utilization management functions in the outpatient setting. Ensure effective care planning and resource allocation for high-risk, high-utilization patient populations. Establish and monitor performance indicators, quality metrics, and financial outcomes related to outpatient care management.
  • Collaboration and Integration: Serve as a system-level liaison between outpatient care management teams, primary care, specialty care, inpatient care management, and community-based resources. Build strong relationships with payers, community organizations, and post-acute providers to optimize care transitions and reduce gaps in care. Partner with IT and analytics teams to leverage population health tools, data, dashboards, and predictive modeling to support proactive care management.
  • Team Development: Provide leadership coaching, and professional development for care management leaders and staff. Promote a culture of accountability, continuous improvement, and patient-centeredness. Ensure recruitment, retention, and training strategies support high-performing interdisciplinary teams.
  • Regulatory Compliance: Ensure compliance with all regulatory, accreditation, and payer requirements to outpatient and population health care management. Maintain current knowledge of healthcare trends, value-based programs, and best practices in outpatient care management.
  • Performance Metrics: Reduction in avoidable ED visits, hospital readmissions, total cost of care. Improvement in patient engagement, care plan adherence, and satisfaction scores. Achievement of quality and cost targets in value-based contracts. Standardization and scalability of care management processes across the system.
Qualifications
  • 5-7 years of relevant experience
  • Master's Degree in Social Work, Public Health, Healthcare Administration, or Bachelor’s Degree in Nursing is required
  • Licenses And Certification:
    Current Maryland RN License, or Social Worker, Counselor or Psychologist License
  • Preferred:
    Care Management Certification within 3 years of hire (CCM - Certified Case Manager)
