We are seeking a dedicated, experienced, and high performer Physical Therapist
(PT) to join our team. The Physical Therapist will evaluate, plan and administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
- Assessment and Evaluation:
Conduct thorough assessments and evaluations of clients’ physical functioning, mobility, strength, balance, and pain levels to develop personalized treatment plans. - Treatment Planning:
Develop individualized treatment plans based on clients’ specific needs, goals, and medical history, utilizing evidence-based physical therapy techniques and interventions. - Therapeutic Interventions:
Provide hands-on therapy, therapeutic exercises, manual therapy techniques, gait training, and modalities to improve clients’ functional abilities and maximize independence. - Patient
Education:
Educate clients and their caregivers on therapeutic exercises, home exercise programs, injury prevention strategies, and adaptive equipment to promote self-management and optimal outcomes. - Progress Monitoring:
Monitor clients’ progress regularly, adjust treatment plans as needed, and document objective data, subjective observations, and treatment interventions accurately and timely. - Collaboration:
Collaborate effectively with other members of the healthcare team, including physicians, nurses, occupational therapists, speech therapists, and social workers, to coordinate care and achieve optimal outcomes for clients. - Communication:
Maintain open and effective communication with clients, caregivers, and healthcare providers to ensure continuity of care, address concerns, and facilitate smooth transitions between care settings. - Compliance:
Adhere to all regulatory and documentation requirements, including completing timely and accurate documentation of evaluations, progress notes, and discharge summaries in accordance with Medicare guidelines and agency policies.
- Master’s, or Doctorate degree in Physical Therapy from an accredited program.
- Current valid state Physical Therapy license
- Clinical experience as a Physical Therapist, preferably in a home health or geriatric setting.
- Strong clinical skills and knowledge of evidence-based physical therapy practices.
- Excellent interpersonal, communication, and problem-solving skills.
- Ability to work independently and manage a diverse caseload effectively.
- Reliable transportation and valid driver’s license.
- CPR certification (BLS for Healthcare Providers).
- Current TB testing (must be completed within a year of the application)
