Med Star Health is looking for a Community Health Advocate II to join our team. The role requires a Certified Community Health Worker with personal knowledge of the target population, residency in the neighborhood, knowledge of local resources, and strong advocacy and community capacity building skills. The advocate will work closely with community health, case management, care teams, and social services agencies to provide short‑term care coordination and connect patients to resources and programs.
The position operates in both clinical and community‑based settings, including patients’ homes, serving as a liaison between health/social services and the community to facilitate access to services and improve quality and cultural competence of service delivery. Responsibilities include outreach, community education, informal counseling, social support, navigation support, and advocacy, as well as conducting home visits, assessments, care coordination, and health screenings.
Duties
- Advocate for individual and community health equity, assist patients in accessing health‑related services, and help them navigate barriers to care.
- Collect, track, and report information about the community and community benefits; document all patient encounters and submit monthly reports; maintain comprehensive electronic client files, including client notes, releases of information, assessments and other medical documents.
- Educate clients on the proper use of Emergency Room/Health system services, provide alternatives, coach patients in effective management of chronic health conditions, teach care plan understanding, and motivate patients to be active participants in their health.
- Enhance community residents’ ability to communicate with health care providers, address barriers to care, and integrate with the patient care team to support progress in the care plan.
- Provide care, support, follow‑up, and education in community settings; deliver culturally and linguistically appropriate health education, evidence‑based guidance, and social assistance; coordinate referral and follow‑up services for human services options.
- High School Diploma or GED.
- Experience as a community health advocate.
- Certified Community Health Worker in the State of Maryland.
- Residency within the target population preferred.
- Participation with a community organization, verified by personal or professional reference.
This position has a hiring range of USD $23.65 – $42.03 per hour.
