Clinical Pharmacist

Paid Range

Base pay: $/yr - $/yr (Actual pay based on skills and experience).

Responsibilities

Perform medication order processing responsibilities within established policies, procedures and guidelines and provide drug information while applying a thorough understanding and knowledge of drug and medical therapy.

Functional Accountabilities Medication Processing
  • Interpret medication orders, profile orders and oversee the packaging, labeling and distribution of medications.
Drug Intervention and Education
  • Perform drug therapy monitoring and intervene as necessary on drug dosing, drug administration, drug interactions, side-effects and monitoring issues; complete therapeutic drug monitoring consults and adverse drug reaction reports as assigned.
  • Provide accurate and timely drug information to physicians, nurses and other health professionals.
  • Interpret research medication orders, ensure accurate preparation of investigational drugs and maintain complete and accurate records.
  • Maintain proficiencies in medication handling (including hazardous medications), compounding sterile/non-sterile preparations and completing quality assurance checks per standard operating procedures and policies in compliance with USP , and guidelines.
Supervision/Training
  • Oversee the work of Pharmacy Technicians by directing and assisting as necessary to ensure rapid medication order turn-around time.
  • Assist with the education, training and general supervision of Pharmacy Residents, Students and Technicians.
Safety
  • Speak up when team members appear to exhibit unsafe behavior or performance.
  • Continuously validate and verify information needed for decision making or documentation.
  • Stop in the face of uncertainty and take time to resolve the situation.
  • Demonstrate accurate, clear and timely verbal and written communication.
  • Actively promote safety for patients, families, visitors and co-workers.
  • Attend carefully to important details - practicing Stop, Think, Act and Review in order to self-check behavior and performance.
Qualifications
  • Completion of a five year pharmacy program in an accredited college of pharmacy. (Required)
  • 1 year Pharmacy experience. (Required)
