The Entitlements Case Manager role is a multi‑entity position covering three provider sites:
Lighthouse Behavioral Health Center, Community Wellness Outpatient Mental Health Center, and Community Wellness Psychiatric Rehabilitation Program. The position may be performed either as a 1099 independent contractor (≈ 40 hours/week, fee‑for‑service) or as a W‑2 employee (≈ 40 hours/week, fee‑for‑service). The incumbent will coordinate services, manage benefits navigation, and maintain regulatory compliance across all three organizations.
- Conduct intake assessments and develop individualized case plans for clients in community‑based group homes, office locations, and via telehealth.
- Navigate and maintain eligibility for Medicaid, SNAP, SSI/SSDI, TCA, housing, and other support services; track applications and follow‑up on recertifications.
- Coordinate with external providers (housing, probation, employment, education, medical) to address social determinants of health and provide crisis/safety interventions as needed.
- Maintain accurate documentation in the electronic health record (ICANotes) and provide necessary compliance reports (CARF, Medicaid, COMAR).
- Collate invoices, track time, and deliver invoices per agency requirements for independent contractors; submit weekly invoices for employee team members.
- Participate in interdisciplinary team meetings, audits, quality improvement initiatives, and professional development training.
- Support backup coverage or fill staffing gaps for primary PRS/HHCM staff as requested.
- Bachelor’s degree in Psychology, Social Work, Counseling, Nursing, or related field (Master’s preferred).
- Valid Maryland license or certification:
LCADC, LGADC, CAC‑AD, CSCAD, ADT, LCPC, LGPC, LCSW‑C, LCSW, LMSW, LBSW, LCMFT, LGMFT, Registered Psychologist, Clinical Psychologist, CRNP‑PMH, APRN‑PMH, CNS‑PMH, or eligible student intern. - 2+ years of behavioral health, case management, or health‑home experience; experience in co‑occurring or dual‑diagnosis populations preferred.
- Strong documentation skills, familiarity with ICANotes/CRISP, and knowledge of COMAR, Medicaid, and CARF standards.
- Trauma‑informed, culturally responsive practice; ability to work independently and collaboratively.
- Will maintain professional liability insurance and provide proof upon request.
Annual salary: $50,000‑$100,000 (fee‑for‑service). Benefits include PTO, family/medical leave, health/dental insurance, retirement plan, and professional development opportunities. Eligibility for in‑office or hybrid setting with 10% travel.
Compensation & Fees (1099)Fee‑for‑service range: $30.00‑$45.00 per hour (credentialed hours). No employee benefits; contractor responsible for taxes, insurance, and licensing costs. Payments issued Net 30 upon invoice submission.
