This is an administrative support position with regular interaction with ACT clients, natural supports and other ancillary service providers (e.g. landlords, social security). The Administrative Specialist for the ACT team manages the day-to-day operations of the team, data collection and management and serves as first point of contact.

Principle Responsibilities and Duties
  • Facilitate daily operations for the ACT team in a supportive manner
  • Assure documentation of daily team meetings to include recording status of each client; planning staff daily schedules, client weekly/monthly services; planning for future services and team members present
  • Provide direct support to staff including monitoring and coordinating daily team schedules and supporting staff both in the office and field
  • Serve as liaison between clients and staff, such as attending to the needs of walk-ins and calls from clients/natural supports
  • Main team roster including client to staff ratio, services being provided from team (employment, co-occurring, housing, etc.)
  • Manage data for reporting and monitoring fidelity status
  • Monitor and audit enrollment documentation and time lines
  • Monitor and audit creation of and implementation of Transition Plans
  • Actively participate in daily team meeting
  • Audit medical records for completion of required documentation
  • Monitor client entitlement status; coordinate with Billing, Entitlements and other support staff as needed
  • Observe and report any changes in clients’ conditions, physical and/or mental health
  • Manage difficult or emotional individuals situations, respond professionally for requests for assistance and support and meets commitments to clients and to team
  • Assess for health and safety and take action with individuals, groups and environment
  • Respond to crisis situations
  • Serve as an advocate for individuals served
  • Develop and maintain professional relationships with individuals, their support system, other staff and community agencies to provide coordinated and comprehensive care
  • Plan coverage for absences in conjunction with your supervisor
  • Complete training and documentation as required by regulation and accreditation bodies
  • Perform other duties as assigned
Other Responsibilities
  • Assist with maintenance of program space, equipment and supplies
  • Participate in improving program effectiveness and efficiency by contributing to program evaluation, as well as working to ensure continuous improvements in quality of services
Knowledge, Skills, and Abilities Required
  • Requires HS Diploma or equivalent plus two years of relevant work experience.
  • Proficient in Microsoft Office suite of products, ability to navigate unknown applications, proficient with keyboarding skills to input required documentation into electronic health record.
  • Safe Driving Record.
  • Must be covered under an active auto insurance policy and valid copy of auto insurance is required prior to hire.
  • Must have reliable transportation plan; may be required to drive to meetings, local business or to support team in the community.
