NHSCT Senior Nurse Permanent Placement Team

Spruce House, Braid Valley site Ballymena.

The post holder will be a highly skilled practitioner providing expertise in care, advice and support within Permanent Placement Team and care homes enhancing the residents experience by supporting the care home staff with their knowledge and skills.

They will support PPT to delivers a high quality service for adults who permanently reside in Care Homes.

They will respond effectively and flexibly to service users’ needs and their carers through an integrated approach including the interface between Statutory and Support Services within identified care home facilities.

The post holder will lead through example, demonstrating the highest standards of practice, informed by relevant legislation, departmental policy, Trust procedures, objectives and current research.

• Collate information from a range of relevant sources to inform decision-making in relation to assessment of risks, needs,care planning, service provision,monitoring and review.
• Participate in and chair if required multidisciplinary/multi-agency reviews and case conferences as appropriate.
• Responsibility to ensure the direct discharge of Statutory and legislative functions have been carried out.
• Lead in supporting and providing a person-centred holistic approach to assess the mental, physical, psychological and social wellbeing abilities and needs of the person, while in Permanent care.

• Registered Nurse: first level, on the live NMC register.
• Have a minimum of 4 years post registration nursing experience, with 3 years at Band 6 or above.
