RN Case Manager
Start Date:
3/30/2026 Pay Rate: $1800.00 - $2700.00
Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) in an inpatient setting.
MINIMUM REQUIRED QUALIFICATIONSCA RN license; BLS; 3 years' nursing experience in an acute hospital setting;
Prior care coordination experience in a Common Spirit/Dignity/CHI hospital
