Base pay in range will be determined by applicant’s skills and experience. Role is also eligible for team-based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities
  • Conduct and coordinate the development and completion of resident assessments in accordance with state, federal, and company guidelines.
  • Coordinate the development and completion of MDS, including CAAs and Triggers; conduct interviews; evaluate resident condition and medical data; develop care plans; ensure timely transmission of data.
  • Develop, implement, and maintain an ongoing quality assurance program for resident assessment and care plans.
  • Assist the resident and discharge planning coordinator in completing care plan portions of discharge plans.
  • Participate in community surveys and inspections by authorized government agencies.
  • Provide leadership and participate in committees such as Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
  • Develop and conduct in-service training classes on assessment skills and MDS functions; assist the in-service director in training activities.
Qualifications
  • Must possess a Nursing Degree from an accredited college or university.
  • Minimum 2 years of supervisory experience in a hospital, nursing care community, or related health care facility.
  • Minimum 6 months of training experience in rehabilitative and restorative nursing practices.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices, regulations, and guidelines governing long‑term care.
  • Current state RN license.
  • Uses limited independent judgment to make decisions based on established guidelines.
Knowledge & Skills
  • Working knowledge of essential nursing skills and disciplines.
  • Overall understanding of the work environment and processes.
  • Working knowledge of the organization.
Physical Demands
  • Standing, walking, sitting, walking with hands and arms, lifting up to 50 pounds.
  • Interaction with co‑workers, residents or vendors.
  • Potential exposure to communicable diseases, blood‑borne pathogens, and latex.
  • Climb or balance; stoop, kneel, crouch, or crawl.
  • Use of hands and fingers to handle or feel; reach with hands and arms.
  • Talk and hear; taste or smell.
  • Exposure to circumstances such as exposure to bodily fluids, injury from falls, burns or cuts from equipment.
  • Vision required.
