Key Responsibilities :
As a Home Health RN, you will:

• Conduct Home Health Nursing Assessments to determine equipment needs, participant preferences, and care goals.
• Deliver and document home health nursing interventions in alignment with care plans, including medication administration, physician orders, and maintaining a safe environment.
• Educate participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
• Track and monitor Home Health scheduling in collaboration with the Home Health Manager or scheduling point of contact.
• Work closely with Physicians and the Interdisciplinary Team (IDT) to ensure smooth care transitions between hospitals, skilled nursing facilities, and home settings.

Basic Qualifications:

• Graduate of an Accredited School of Nursing.
• Unencumbered RN License required.
• Minimum one year of nursing experience in a clinical setting, preferably with a frail or elderly population. Home health agency experience is a plus.
• Reliable transportation for home visits.
