• Associate degree in a social science or related field required.
• Bachelor’s degree in a social science or related field preferred.
• Two (2) years’ experience in a mental health setting preferred.
• Experience with case management services preferred.
• Valid CPR certification required within 30 days of employment.
• Valid First Aid certification required within 30 days of employment.
• Valid Pro Act certification required within 30 days of employment.

• Has responsibility for processing referrals to outpatient/community-based treatment providers.
• Participates in initial assessments and obtains outpatient treatment provider information, options, and resources.
• Verifies follow-up appointments and collaborates with follow up treatment and step-down services. Provides outreach with caregivers, county social workers, and outpatient/community-based service providers regarding patient progress.
• Conducts groups to assist patients with transition to outpatient/community-based services.
• Collaborates with PHF staff and families/caregivers to plan for and schedule discharge.
• Submits discharge packets to referred agencies.
• Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established.

• 24/7 inpatient involuntary program
• 16-bed facility for acute youth (ages 5-17)
• Intensive treatment programs with individualized care plans
• Available for contracting Central Valley counties
• Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
