Job Details- Job Title: Case Manager
- Division/Program: (Children/Youth Inpatient) Psychiatric Health Facility
- Starting Compensation: 22.00 - 28.00 USD Per Hour

Bakersfield, CA - Working Hours/Shift: Tuesday-Saturday 9:30AM-6:00 PM

What You Bring To SBHG- Associate degree in a social science or related field required.
- Bachelor’s degree in a social science or related field preferred.
- Two (2) years’ experience in a mental health setting preferred.
- Experience with case management services preferred.
- Valid CPR certification required within 30 days of employment.
- Valid First Aid certification required within 30 days of employment.
- Valid Pro Act certification required within 30 days of employment.
- Has responsibility for processing referrals to outpatient/community-based treatment providers.
- Participates in initial assessments and obtains outpatient treatment provider information, options, and resources.
- Verifies follow-up appointments and collaborates with follow up treatment and step-down services. Provides outreach with caregivers, county social workers, and outpatient/community-based service providers regarding patient progress.
- Conducts groups to assist patients with transition to outpatient/community-based services.
- Collaborates with PHF staff and families/caregivers to plan for and schedule discharge.
- Submits discharge packets to referred agencies.
- Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established.
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 5-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare

For Additional InformationIn accordance with California law, the grade for this position is 20.20 - 30.30. Placement within the grade is determined based on experience, internal equity, and other factors permitted by law.
