** Certified Occupational Therapy Assistant (COTA) – PRN*
* * Starting Rate: $45/hour
* Sequoia Transitional Care, a skilled nursing facility, is seeking a dedicated
** PRN Certified Occupational Therapy Assistant (COTA)
** to join our rehabilitation team. This role provides flexibility while making a meaningful difference in the lives of our residents.###
** General Purpose
** The COTA assists the Occupational Therapist with patient-related activities and direct patient care, helping residents achieve their highest level of independence and quality of life.###
** Essential Duties
*** Provide therapy treatments as directed by the Occupational Therapist.
* Record daily treatment notes and weekly progress notes in compliance with OT Board standards.
* Follow physician treatment plans for each patient.
* Assist nursing staff with training Restorative Aides.
* Communicate effectively with the supervising OT and interdisciplinary team regarding patient progress and plans.
* Participate in in-service training programs for facility staff.
* Maintain accurate documentation including evaluations, recertifications, and discharge notes per state and payer requirements.
* Provide caregiver training and family education to support safe discharge planning.
* Assist in maintaining a clean and safe treatment area, reporting equipment issues promptly.###
** Qualifications
* ** Current
** COTA license
** in the state of California.
* Proficient in electronic documentation systems (e.g., Casamba, Point Click Care ).
* Strong communication skills with patients, families, and healthcare team members.
* Ability to read, interpret, and apply therapy standards, policies, and physician orders.
* Must meet ongoing state continuing education requirements.###
** Physical Demands
*** Frequent standing, walking, and reaching.
* Ability to push/pull and lift up to 50 lbs frequently and 100 lbs occasionally.
* Occasional stooping, kneeling, or crouching.
* Visual acuity including close, distance, and peripheral vision.###
** Work Environment
*** Skilled nursing and rehabilitation setting.
* Low to moderate noise levels.
