About

The Role

We are seeking a compassionate and motivated Associate Mental Health Practitioner to join our Older People’s Response Team in Bradford. This is a community-based role, supporting older adults across a wide area, including visits to patients’ homes and community settings. You will play a key part in delivering person-centred, holistic care, supporting cognitive and physical health assessments, and promoting healthy living.

The role also involves running therapeutic groups, working closely with service users, families, and carers, and collaborating within a multidisciplinary team.

About You
  • Able to travel extensively across the Bradford area (access to transport is essential due to the need to carry physical health monitoring equipment).
  • Experience working with older people and/or in community mental health settings is desirable.
  • Excellent communication and interpersonal skills, with a person-centred approach.
  • Able to work both independently and as part of a team.

  • To support the Older Peoples Response Team in undertaking detailed assessments of ability for service users accessing the team.
  • To undertake physical health assessments and reviews of service users and promote healthy living and lifestyle.
  • Support the delivery of therapeutic interventions.
  • Working alongside service users, family members and carers to provide signposting, support and advice.
  • Working in an interdisciplinary way to provide person-centred holistic care and promote independence for individuals.
  • To work together as part of the Multi-Disciplinary Team (MDT) to support the delivery of specialist assessment, for service users referred to our dementia outreach service under the care of our OPMH service, reflecting the needs of the population.
  • Work autonomously to undertake assessments and deliver interventions aligned to the key professional roles within the OPMH MDT with the aim of optimising the health and wellbeing of individuals: and do so with own scope of practice.
  • To seek appropriate support and guidance when required from other suitably qualified professionals.
  • To keep up to date clinical records and provide relevant data regarding service user outcomes and clinical resource management.
