• Conduct comprehensive geriatric assessments as part of the older people’s pathway, identifying early signs of frailty.
• Collaborate with an inter‑agency, inter‑disciplinary team to provide specialised assessment and care planning for older people with frailty.
• Deliver assessment, clinical care and management plans for both stable and acutely unwell older individuals or those with chronic conditions.
• Facilitate alternatives to hospital admission where clinically appropriate, and support early hospital discharge.
• Work closely with hospital‑based teams to assess, plan, and deliver care for vulnerable patients experiencing complex health and social issues.
• Promote and develop community alternatives to hospital admission and provide support for discharge.

• Qualified physiotherapist with a degree or equivalent in Physiotherapy.
• Recent UK experience in specialised physiotherapy is essential.
• Registered with the HCPC as a physiotherapist to work in the UK.
• NHS experience is advantageous but not essential.
