Job SummaryThe Discharge Supervisor is responsible for overseeing all aspects of discharge for patients receiving services in Crisis, Inpatient, or Transition Point programs. The role involves active participation in patient discharge planning from admission, engaging family members, enrolled providers, and other identified supports, attending 4‑hour staffings and daily huddles with the multidisciplinary team to identify potential discharge barriers, and ensuring the discharge transition plan accurately reflects placement details, referrals, and follow‑up appointments.
The position works closely with the Clinical Manager, Clinical Director, and Operations team to ensure discharge planning and services are effective, efficient, and compliant with CBI Clinical Policy, Arizona Administrative Code, ADHS/DBHS Provider Manual, Code of Federal Regulations, and contract requirements.
- Bachelor’s Degree in a related field (Master’s Degree preferred).
- 1–3 years of full‑time behavioral health care related work experience; prior experience in both behavioral and medical crisis situations is preferred but not required.
- Fingerprint clearance card required.
- Current AZ driving license.
- Clear 39‑month motor vehicle record.
