Job Description
Provides direct Physical Therapy (PT) treatment under the direction of the Physical Therapist in accordance with the physician’s orders and as outlined in the PT Plan of Care.
Monitors patient’s status and home environment to ensure the patient is successfully completing treatment as delegated by the registered Physical Therapist.
Reports to the registered Physical Therapist any changes in the patient’s condition and/or needs.
Maintains communication with other agency personnel involved in the patient’s care to promote coordinated care.
Prepares, submits, and maintains accurate, relevant, and timely clinical documentation per agency policy.
Attends mandatory agency staff meetings, care conferences, and educational offerings.
Performs administrative tasks such as keeping records, writing reports, and making telephone calls.
Maintains current CPR certification, Tuberculosis, and all other required licensures.
Assumes responsibility for personal growth through continuing education and stays current with changes in home care, seminars, and membership in professional organizations.
The Physical Therapist Assistant performs skilled direct patient care activities under the clinical supervision of the Physical Therapist.
