• Discharge planning
• Chart review and patient screening
• Care Coordination
• Barrier mitigation in an acute hospital setting

TX/compact RN license; BLS; 2 years case manager experience in an acute hospital setting;
Acute hospital experience during the past 12 months with no more than a 90-day break

8:30A-5:00P. Potential rotating weekends.
