Job Summary and QualificationsThe MCS and Shock Coordinator is a registered or advanced practice nurse whose primary function is to augment the capability of the MCS and Cardiogenic Shock program in effective management and delivery of quality care to patients presenting with heart failure and/or shock. This role involves 24-hour accountability for clinical practice, coordination of resources, program development, and standards compliance in a rotational on-call coverage.
This role participates in delivery of patient-specific care, as directed, and fosters service excellence standards to ensure high levels of patient satisfaction across the market.
- Coordinate program development for MCS (Impella, RVAD, LVAD, ECMO) and Shock care
- Attend and actively participate in facility meetings.
- Navigate shock patients for care escalation, extracorporeal life support, and advanced therapies.
- Coordinate care for MCS and shock patients, both within and external referrals into St. David’s Health Care facilities.
- Plan, coordinate, and implement device specific care for pre/post-VAD patients, in both inpatient and outpatient settings.
- Coordinate critical care transport of MCS, ECMO, and Shock patients in collaboration with patient transfer center, ambulance agencies, transport team, accepting/sending provider, and hospital leadership.
- Evaluate patients for candidacy for MCS, and track and coordinate necessary testing.
- Communicate and collaborate within the multidisciplinary MCS and Shock teams.
- Serve as a liaison for heart failure and shock patients for other care providers.
