• Perform surveyor duties to accrediting or certifying facilities serving residents of long-term care facilities.
• Demonstrate independent decision-making skills to direct and effectively manage the survey process.
• Set priorities independently and collaboratively in performing survey tasks.
• Openly discuss conflicts, seek assistance when appropriate, and resolve conflicts to make informed decisions.
• Travel up to 75% of the time on a regular basis to perform duties.

• Master’s degree in Social Work.
• CMS Long-Term Care Basic Training completed and (SMQT) Surveyor Minimum Qualifications Test passed.
• At least two (2) years of recent experience as a surveyor accrediting or certifying facilities serving long-term care residents.
• Current licensure to practice as an LCSW.
• U.S. Citizenship is required for this position.

• Salary: $72,000 – $80,000 per year. Full-time position (Monday - Friday).
