Aveanna Healthcare is seeking Licensed Practical/Vocational Nurses (LPN/LVN) to provide skilled nursing care in patient homes. The role is flexible, covering both day and night shifts with full‑time, part‑time, or PRN options.
Position OverviewThe Licensed Practical Nurse is responsible for delivering and documenting skilled nursing care under the supervision of a Registered Nurse, following care plans and physicians' orders while maintaining confidentiality and professional boundaries.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and report changes to the supervisor/physician.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in and update the nursing care plan.
- Take appropriate nursing action based on assessment to achieve desired outcomes.
- Identify changes requiring intervention and implement care to prevent or reduce risks.
- Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Use infection control measures that protect staff and patients per OSHA standards.
- Educate patients and families about disease processes, self‑care, and prevention strategies.
- Maintain competency through education, continued learning, and skill evaluations.
- Active, unencumbered LPN/LVN license in the state of practice.
- Current CPR certification;
Aveanna can assist in obtaining this after hire if required. - TB skin test within the last 12 months.
- Preferably 6 months of hands‑on nursing experience; not required.
- Reliable transportation.
- South Carolina – at least one year of pediatrics experience.
- California – at least one year of experience working under a nursing license.
- Louisiana – at least one year of experience working as a licensed nurse.
- Continuing education as required by state regulations.

- Strong sense of smell and touch.
- Preparedness to respond physically and mentally to emergencies at home or during transport.
- Work in varied environments, possibly exposed to allergens and other conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases.
