The Clinic Nurse will play a vital role in providing high‑quality, patient‑centered care and support to patients living with HIV and at risk for HIV in our clinic setting. In this role, you will work collaboratively with an interdisciplinary health care team to address both the medical and social needs of our patients. This role requires strong clinical skills, attention to detail, and a commitment to delivering high‑quality, patient‑centered care to a diverse patient population.

  • Conduct thorough patient assessments, including vital signs, medical history, and assessment of current symptoms or concerns.
  • Administer medications and injections as prescribed by healthcare providers ensuring accuracy, safety, and compliance with medication administration protocols.
  • Assist with procedures such as phlebotomy, suture removal, and wound care as needed.
  • Maintain accurate and up‑to‑date electronic health records (EHR), documenting assessments, history, interventions, and patient interactions according to clinic protocols and regulatory standards.
  • Provide education to patients on their health conditions, medications, and self‑care management.
  • Triage and prioritize patient needs, ensuring timely access to care and appropriate referrals.
  • Process patient referrals for specialty care and follow‑up as needed to ensure quality specialty care was provided.
  • Collaborate with medical providers, case managers, and other healthcare professionals, both internally and externally, to coordinate patient care and ensure optimal outcomes.
  • Assist in planning, managing and coordinating patient flow in clinic areas and day‑to‑day operations of the clinic.
  • Contribute to quality improvement initiatives to enhance the delivery of care, identify areas for improvement, and implement evidence‑based practices to optimize patient outcomes.
  • Participate in research projects and continuing education activities to enhance clinical practice and patient outcomes.
  • Perform any and all other duties assigned by the supervisor.
  • Adhere to all agency policies, including Confidentiality, Employee Handbook, Health Care Corporate Compliance Plan, Standards of Conduct, and other relevant policies.
