The Associate Patient Care Coordinator is responsible for staffing all new patient admissions/referrals, scheduling daily patient visits for clinicians, reviewing and submitting end‑of‑day schedules, ensuring completion of all weekend schedules, and reviewing daily schedules with field clinicians. Supports the Clinical Team Manager (CTM) with maintaining daily contact with field clinicians and effectively communicating schedule changes to them. Critical thinking is essential for reviewing weekly active patient census reports, scheduling patient discharge visits with appropriate field clinicians, scheduling patient re‑certification visits, supervisory visits with the primary registered nurse, and appropriate job tasks for field clinicians per physician orders.

Regularly communicates with Intake Department personnel in reference to all new patient referrals and new physician orders for staffing with the appropriate field clinician. Assists with reviewing and processing patient discharge and therapy evaluations. Collaborates with the Director of Nursing, CTM and Director of Home Health Operations to ensure all patients are staffed accordingly per physician orders. Actively participates in the Agency’s on‑call rotation in the Home Health Team Coordinator role, approximately once per month to include holidays.

Schedule:

Monday to Friday, 9 AM — 6 PM and weekly on‑call rotation:
Monday‑Thursday, 6 PM‑8 AM and Friday, 6 PM‑Monday, 8 AM.

Onsite – 8107 Springdale Road, STE 110 Austin, TX 78724.

• Responsible for daily scheduling of all physician orders, new referrals, and schedules with appropriate field clinicians
• Reports new or pertinent patient information to field clinicians
• Schedules per physician order all visits/tasks specific to each patient from the beginning to conclusion of directed patient care: supervisory visits, re‑certification visits, discharge visits, transfers, and post‑hospital visits and ensures accuracy
• Updates clinician schedules frequently to remain in compliance with current physician orders
• Communicates all new physician orders and new referrals with contract therapy staff
• Assists CTMs with contacting field clinicians or relaying information in reference to patient care updates
• Frequently communicates with management regarding unassigned patient visits to ensure all patients receive care within the timeframe required per physician orders
• Responsible for checking email, HALO’s, team messaging system and voicemail to respond to time‑sensitive messages from customers, leadership, lower‑level management
• Responsible for adhering to the company policies to include: time reporting policy, attendance policy, code of conduct, etc
• Cross‑trained for all job tasks in Medical Records to include uploading of documentation in EMR, Start of Care (SOC) audits, reviews contract invoices and Print Queue process
• Prepares clinicians schedules and assigns clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies per physician order
• Demonstrates exceptional customer service to all patients, clinicians, and family members
• Reviews missed visits daily submitted by field clinicians to ensure visits are re‑scheduled accordingly, reviews schedule deviation report for accuracy
• Updates patient information into the EMR when changes occur
