Nurse Practitioner (Skilled Nursing) Avir at Park Bend
Join Avir Health Group as a Nurse Practitioner (Skilled Nursing) in a Skilled Nursing Facility (SNF) setting. You will serve as a clinical leader working collaboratively with the facility’s care team to oversee both short‑term and long‑term residents.
Base pay range$/yr – $/yr
Responsibilities• Admissions, discharges, monthly stability checks, and evaluations for changes in condition.
• Advanced care planning, comprehensive assessments, and prescribing treatments.
• Lead clinical collaboration and support the care team's goals for resident health.
