Intake Counselor - Full Time serves as the first point of contact for referral sources and potential patients to schedule assessments and/or admissions. Responds to incoming calls to the hospital to coordinate referrals/admissions. Provides assessment of patients’ psychiatric and medical status. Ensures all inquiries, referrals, and admissions are accurately documented in the computer system and notes regarding referrals and upcoming admissions are documented on proper forms.
Ensures all referral patient medical record information is completed and nursing and clinical staff are adequately apprised of the patient’s condition/history. Ensures referral material is copied/distributed and prepared in unit charts as required. Assesses, directs, and intervenes in crisis situations. Provides leadership and direction to mental health technicians. Contributes to treatment plans as a member of the interdisciplinary team. Provides and directs milieu management.
Delivers health education to patients and families. Maintains confidentiality of patients’ medical records. Communicates with sensitivity to growth, development, and cultural issues. Attends mandatory training and in-services.
