Overview
Under the supervision of the Sr Associate Dir of Nursing, the Associate Director of Nursing - Case Management is responsible for care coordination, clinical team formation, and daily operations of case management while supporting the delivery of direct patient care for low-income and uninsured patients in community environments as appropriate. This position will have direct oversight over personnel within their scope.
The AD of Case Management will work with other departments, such as nursing, to ensure that all standards of care are metregarding each specialty.
The Associate Director of Nursing - Case Management will work in conjunction with Registered Nurse Case Managers, Social Workers, Community Health Workers, Physicians, and other care team members to provide case management and navigation services while working in collaboration with inpatient care teams and other Central Health medical and transitions of care teams.

This is an onsite role located in Austin, Tx. You must live in the greater Austin Metropolitan area to be considered for the role.
ResponsibilitiesEssential Functions:
- Facilitate collaboration with partner organizations to ensure the provision of compassionate and effective care coordination to hospitalized patients and medical care to other patients in post acute settings who require case management services and needs upon discharge.
- Organize and lead operational, clinical and quality care conferences across the enterprise with internal and external stakeholders to educate on services available to specified case management populations.
- Work in close consultation with the patient's primary care provider and other licensed health care facility provider teams to coordindate the medical care of MAP patients when discharged from hospitals, post-acute settings and referred from specialty care and external providers.
- Communicate essential information to patients, families, colleagues, nursing and other health care professionals, as appropriate.
- Prepare and provide necessary timely and accurate reports and forms, as may be required by Central Health or facility in the performance of case management services.
- Perform clinical duties as assigned and competency validated in accordance with Standing Delegation Orders including, but not limited to: medication reconciliation, assisting provider with examination in community settings where applicable and patient homes, cleaning and disinfection of equipment, perform quality control checks on equipment as per manufacturer guidelines patient vital signs, glucose monitoring and education, and phlebotomy as outlined in Care at Home SDO.
- Plan and coordinate care daily with all members of Central Health's care team to assure maximum quality and efficiency of care between Eligible Patients, Physicians, Advanced Practice Providers, Case Management and nursing.
- Perform medical and administrative services under general guidance and minimal supervision with accountability for specific organizational-level goals.
- Under the oversite of the Director of Case Management, align clinical and non-clinical resources to organizational goals and objectives to optimize the health of the covered population and improve clinical outcomes, patient experience and costs of care throughout the delivery system.
- Participate in the development, deployment and management of safe, effective and efficient clinical services to Central Health's covered population through a variety of methods including clinical support infrastructure development and oversight of case management services as applicable and appropriate pursuant to the Medical Executive Board's policies and procedures.
