Overview

The Case Worker - Substance Use is responsible for ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective. This position operates in a dynamic environment and performs case management, assessment, and/or service coordination work based on program guidelines. Duties may include providing continuity of care services, linking consumers and families to needed resources, and coordinating, monitoring, and updating services and consumer goals.

Additional roles include providing needed skills training, crisis prevention, and community outreach. As a team member of programs in substance use services, the Case Worker coordinates clinical and substance use treatment tools to address client needs, utilizing assessment, multidisciplinary coordination, and efficient, clinically appropriate use of agency resources. The position requires extensive knowledge of substance use disorders and working knowledge of treatment techniques effective with chemically dependent clients, especially those with history of opiate use, and a working knowledge of methadone/buprenorphine treatment.

A working knowledge of infectious diseases such as Hepatitis, HIV, STD and other infectious diseases is required.

This position follows identified strategies for achievement of unit performance measures and ensures the highest quality of care while effectively utilizing resources.

This position may carry an individual caseload. On-call responsibilities may be required depending upon unit protocols. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.

Regular, predictable attendance is considered an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education
  • High School Diploma required.
  • Graduated from an accredited college or university with a Bachelor’s degree with a major specialization in Human Services or related field preferred.
License
  • Current and valid driver’s license and a clean driving record required.
  • LCDC or LCDC-CI required. Must maintain licensure throughout employment required.
Experience
  • 1-2 years in direct care in mental health/SUD services preferred.
  • Bilingual preferred.
Hours

Full-Time.

Workdays

Monday - Friday.

Workhours

6:00AM - 3:00PM.

Salary

$23.03 - $35.68 an hour depending on years of licensure, plus an additional 10% crisis stipend.
