Under the supervision of the Director of Specialized Health and Social Services, and with oversight by the Regional Team Leads/Supervisors, this position provides advanced and comprehensive case management consultation for families with children who have health risks or special health care needs. The individual will attend work on a regular and predictable schedule in accordance with agency leave policy and perform other duties as assigned.
Key Responsibilities- Oversee case management services for a broad array of programs including CSHCN, Medicaid, CHIP, Title V, and others.
- Assist families with eligibility determination and conduct comprehensive assessments to determine the need for PCS/PDN, CSHCN, and CPW services.
- Travel to conduct home visits, complete intakes, family needs assessments, service plans, and follow‑ups.
- Link families to community resources and communicate program services positively.
- Participate and advocate in Community Resource Coordination Groups (CRCG), case management coalitions, interagency groups, and community organizations.
- Complete administrative duties, monthly reports, special assignments, and work activities in a professional and timely manner.
- Serve in supporting roles for disaster response, recovery, or Continuity of Operations (COOP) activation as required.
- Maintain confidentiality and work under minimum supervision with significant initiative.
- (35%) Case Management Functions: Provide complex and specialized consultation, conduct home visits, maintain documentation, and use software for database entry and reporting.
- (25%) Comprehensive Assessments: Determine eligibility, document needs, enter information into online database, conduct reassessments.
- (20%) Community Outreach/Resource Development: Collaborate with community entities, represent DSHS in CRCG and other groups, maintain a resource directory, provide presentations.
- (10%) Professional & Team Development: Attend training and conferences, maintain licensure, continue professional development.
- (10%) Additional Functions: Participate in performance evaluations, staff meetings, and other assigned duties.
- Social work and case management principles and best practices.
- Children’s programs such as Medicaid and Early Childhood Intervention, plus community resources at local, state, and national levels.
- Cultural and socioeconomic issues related to health and disability.
- Maternal, infant, and children’s health, including mental health.
- Skill in developing and presenting information to diverse audiences.
- Proficiency with electronic mail, internet, databases, and Microsoft Office products.
- Problem solving, planning, prioritizing, decision making, and organizing workload.
- Effective verbal and written communication.
- Adherence to the National Association of Social Workers’ Code of Ethics.
- Provision of own transportation for home visits.
