We are looking for a registered PT professional who provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being. Plans organizes and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary teams.

- Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and determine the level of functioning and appropriateness of patient for home care.
- Develops and evaluates the care plan in partnership with the patient, representative (if any), and caregiver(s).
- Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
- Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, to perform patient care that is consistent with patients’ needs.
- Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Accepts clinical assignments that are consistent with education and competence to care for patients. Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
- Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
- Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on the use of equipment such as walkers, wheelchairs, canes, etc., in a safe manner.
- Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
- Provides patient, caregiver, and family counseling;
Provides patient and caregiver education. - Uses effective interpersonal relations and communication skills.
- Licensure: Active, state license as a physical therapist or a multi‑state license issued by a Physical Therapy Licensure Compact (PTLC) member state.
- Experience: 1 year of experience as a physical therapist in an acute care, rehabilitation, or home health setting is preferred. The client is willing to train.
Skills: Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification. Knowledge of durable medical equipment.
EMR: Kinnser experience preferred but not required.
