Relief Habilitation Coordinator - QMHP at Integral Care-Austin, TX
Job SummaryThe Relief Habilitation Coordinator works alongside a team of advocates to help adults with Intellectual and Developmental Disabilities (IDD) seeking admission to a Medicaid-certified nursing facility. The role follows federally mandated PASRR guidelines, screens people for appropriate placement, and coordinates state electronic systems such as CARE and TMHP. Coordinators use a person‑centered approach to perform assessment, service planning/coordination, monitoring, and crisis prevention/management within a multidisciplinary team.
Responsibilities• Perform PASRR screening and resident review activities.
• Facilitate transition duties when an individual requests to move out of the nursing facility.
• Apply assessment, service planning, monitoring, and crisis prevention/management elements.
• Participate in a multidisciplinary team with internal and external community partners.
• Maintain regular, predictable attendance and a flexible work schedule as required.
• Foster a trauma‑sensitive workplace and supportive environment of care.
- Education
:
Bachelor’s degree in Human/Behavioral/Social Services or related field from an accredited college or university. - License
:
Current and valid driver’s license with a clean driving record. - Experience
:
Minimum one year of direct experience working with individuals with DD. - Ability to work Monday through Friday, 8:00 AM – 5:00 PM.
- Knowledge of state PASRR, CARE, and TMHP electronic systems is preferred.
Full‑time, Monday – Friday, 8:00 AM – 5:00 PM. Hourly rate: $24.18 – $25.39 based on education.
